Job Description:
A company is looking for a Care Manager (RN) to provide
telephonic care management services for members with chronic or
complex diagnoses.
Key Responsibilities
Evaluate member needs and develop personalized care plans
Coordinate care between members, families, and care providers to
ensure appropriate services
Monitor member progress and update care plans as necessary
Required Qualifications
Degree from an Accredited School of Nursing or Bachelor's degree in
Nursing
2 - 4 years of related experience in care management or similar
settings
RN - Registered Nurse licensure required (state or compact)
Experience caring for individuals with chronic/complex
diagnoses
Preference for candidates located in the eastern time zone
