Job Description Job Description Job Purpose The purpose of this role is to coordinate and provide transportation services for CSC PACE participants. Responsibilities include receiving incoming transportation requests, making outgoing coordination calls, and managing the daily transportation of participants. The position supports contracted transportation vendors (e.g., Agape) by facilitating communication and arranging back-up services as needed. Duties include, but are not limited to, scheduling, route planning and mapping, documentation, recordkeeping, and auditing activities to ensure compliance with CSC PACE policies and procedures and applicable regulatory requirements. Duties and Responsibilities Receive and manage incoming transportation requests from PACE participants and interdisciplinary team (IDT) members; place outgoing calls related to transportation coordination. Schedule, coordinate, and maintain daily transportation routes for PACE participants in accordance with individualized care plans. Monitor daily routes to maintain real-time communication with drivers and proactively address delays, service disruptions, or participant needs. Develop, map, and modify transportation routes for new enrollments and existing PACE participants to ensure timely and efficient service delivery. Coordinate daily transportation services to and from PACE centers, medical appointments, and approved destinations, ensuring continuity of care through collaboration with the IDT, PACE staff, and drivers. Ensure all scheduled transportation services are assigned, covered, and completed by the end of each business day. Coordinate with clinic and PACE center staff to arrange transportation for urgent medical needs, including same-day appointments, medication deliveries, and durable medical equipment (DME). Prepare and distribute daily operational reports or emails to management as needed, documenting service issues, delays, incidents, or participant complaints related to transportation services. Coordinate with contracted transportation vendors to ensure coverage for open, overflow, or specialized transportation assignments. Accurately enter, maintain, and export daily transportation and trip data in accordance with PACE documentation and reporting requirements. Review and monitor driver daily logs to ensure regulatory compliance and confirm all assigned trips are executed as scheduled. Monitor usage of transportation gas credit cards, collect fuel and maintenance receipts, and submit documentation to management for reconciliation and audit purposes. Ensure strict adherence to PACE policies and procedures, CMS and DHCS regulations, and all applicable transportation and participant safety requirements. Support program compliance by meeting documentation, reporting, and operational deadlines. Perform other duties as assigned in support of PACE program operations. Qualifications Education: High School or GED diploma Experience Ability to read, write and understand verbal or written instructions. Minimum 3 to 5 years’ experience in health care field to similar work. Skills and Knowledge : Bilingual: English/Mandarin/Cantonese/Vietnamese Must be able to work in a fast paced environment with changing priorities. Must be able to map/route all future trips schedule per day and to reassign open routes per day if necessary including emergency call outs and logistics. Proficient keyboarding skills to enter trip data in real time, able to write 40-50 words per minute (wpm). Knowledge of coordination for medical appointments to and from patient’s home to various medical sites. Communication skills by providing excellent customer service. Coordinating and clerical work by maintaining transportation files up to date. Knowledge of PACE program preferred Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile. Ability to work professionally with other staff member and the general public. Maintain confidentiality Desired Personal Attributes : Dependable, flexible and resourceful. Requires physical strength to perform essential functions of the job. Requires use of personal vehicle. Requires valid driver’s license. Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage: $100,000/$300,000 personal liability and $100,000 property damage. May require use of personal cell phone for business purposes (may be eligible for stipend) Physical Demands Must be able to remain in a stationary position 50% of the time. Ability to occasionally move about inside the office to access file cabinets, office machinery, etc. Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer. Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc. The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.
