Job Description & Requirements - Specialty: Care Manager - Discipline: RN - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Travel Job Description We are seeking an experienced Case Manager for an acute care facility. The Case Manager will coordinate patient care and discharge planning to ensure safe and effective transitions of care. This role is open to first-time travelers; however, candidates must complete a screening process prior to submission. Responsibilities - Coordinate patient care and discharge planning - Develop and implement care transition plans - Collaborate with physicians, nurses, and interdisciplinary teams - Evaluate patient needs and available resources - Ensure timely and safe discharge planning - Monitor patient progress and care plans - Maintain accurate documentation - Communicate with patients and families regarding care plans Requirements - Minimum 2 years of acute care Case Management or Discharge Planning experience - Active California State License required - Pending license accepted (must be active before start date) - First-time travelers accepted (screening required) About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
