Case Manager Behavioral Health Term: Regular Full-Time Industry: Healthcare / Health Services Behavioral Health Job Category: Professional / Case Management Location: Los Angeles, CA Position Summary The Behavioral Health Case Manager is responsible for continuous quality improvement of client care outcomes and the systems that deliver care. The role supports adults with serious and persistent mental illness through comprehensive assessment, service coordination, advocacy, and collaboration with a multidisciplinary treatment team. Key Responsibilities Provide case management services for clients with significant behavioral health needs, including psychosocial assessment, service planning, coordination, and monitoring. Apply knowledge of the special needs and behavioral characteristics of mentally dysfunctional client populations served by the agency. Identify and connect clients and families to appropriate community resources; maintain an up-to-date working knowledge of available services. Ensure adherence to rules, regulations, and ethical/legal standards governing human services and confidentiality of client information. Utilize casework principles, interviewing techniques, and mental health service coordination methods to support client recovery and stability. Document activities, maintain accurate, timely records, and manage information flow required to carry out coordinated service plans. Formulate psychosocial assessments and evaluate community functioning of clients and families to guide service planning. Monitor client adherence to individualized, client-driven coordination plans and adjust services as needed to meet goals. Serve as liaison between the agency and community programs, representing agency goals, policies, and procedures to partner organizations. Advocate for clients and families to promote equity and access to appropriate services, and influence support systems to respond to individual needs. Communicate positively and professionally with clients, families, and community resource representatives, reflecting the agency s values. Identify problems that affect service quality, report them promptly, and assist in developing solutions for improvement. Maintain strict confidentiality in all client matters. Support agency initiatives, including participation in fundraising activities and special projects as assigned. Demonstrate loyalty and commitment to the agency s philosophy, mission, goals, and objectives. Minimum Qualifications Education: Bachelor s degree in Social Work, Psychology, Human Services, or related field (required). Master s degree preferred. Experience: Minimum of 2 years experience in human services as a Case Manager, Social Services Case Worker, Educational Psychologist, or related role in a psychiatric environment with severely and persistently mentally ill clients (preferred). Skills/Competencies: Strong verbal and written communication skills. Ability to effectively work within a multidisciplinary treatment team. Knowledge of community resources and mental health casework principles. Strong interpersonal skills, teamwork, and group dynamics. Ability and willingness to document work and maintain organized records. Physically well and emotionally stable to meet job demands. Schedule Full-time, 8-hour shifts. Work location primarily in the community (on the road), serving clients in various settings.
