Jovenes is a place of personal transformation for homeless youth ages 18-24. Our mission is to help homeless youth and at-risk families become productive and integrated members of the community. We provide emergency and permanent housing options and other community based services that create opportunities for them to lead productive lives. We operate in Los Angeles, East and South East communities of Los Angeles County.In partnership with California State University, Northridge,California State University, Long Beach and Cerritos College Jovenes Inc. agrees to provide housing assistance and co-located housing-related case management to achieve housing security for eligible college students. Funding for this partnership is provided by the State of California, through Assembly Bill 74 (AB-74), College-Focused Rapid Rehousing.JOB TITLE: Assistant Case Manager IMMEDIATE SUPERVISOR: Program Coordinator The primary responsibility of the 'Assistant Case Manager' is to provide support to the Case Manager, in their duties to provide case management, assessments, crisis intervention, and coordinate activities, including supervision of resident housing for Community College residents experiencing homelessness.The goal of the program is to help students develop the appropriate connections and steps in life to become more self-sufficient and independent by accessing permanent housing and or while pursuing higher education.This 'Assistant Case Manager' will provide scattered site through AB 74 as well as on-site support at various shared housing locations for college students experiencing homelessness. DUTIES AND RESPONSIBILITIES Under the supervision of the ' Program Coordinator' , the position will be responsible for the following: PEER NAVIGATION Conduct outreach and process incoming referrals for youth in need from localColleges Assist Transitional Age Students in completing the assessment that is required to enter the CES system; Enter data into databases required for the project; Attend planning and coordination meetings; Outreach to other agencies about housing opportunities and availability. Maintain and monitor CES data systems Attend TAY CES Peer Navigation meetings on behalf of the SPA 7 teamUnder the supervision of the 'Program Coordinator' and the 'Case Manager ', the position will assist with the following: RESIDENTIAL SUPERVISION Provide onsite case management services in the areas of independent living skills, organizing house meetings, social events, mediating roommate disputes, employment linkage, benefits establishment, and other services as needed. Supervise participants at by processing incoming referrals for residents in need from local Community Colleges, assessing youth for skills, challenges and appropriate services, conducting screening interviews, completing intake documentation, coordinating move in and out of all residents, managing supplies, organizing activities, and enforcing program rules. CASE MANAGEMENT AND OTHER DUTIES Support residents in identifying permanent housing options that would help them best reach their educational goals and obtain self-sufficiency. Assist each resident, in implementing their Individualize Service Plan (ISP) that will be updated monthly. Provide youth with referrals and monitor progress as needed, in particular to mental health and substance abuse services and to opportunities for educational & career development and personal enrichment. Maintain files in a secure and locked cabinet and prepare and regularly submit records or when prompted Follow at all times the agency professional code of conduct and appropriate crisis management procedures Coordinate with other program staff in service delivery and facility monitoring. Attend planning and coordination meetings and outreach to other agencies about housing opportunities and availability Respond to information requests and help organize partner meetings Develop and maintain effective relationships with Community College staff and programs Other duties and responsibility may be developed by immediate supervisor according to program needs SKILLS AND ABILITIES REQUIRED TO PERFORM THE ESSENTIAL JOB FUNCTIONS _Communication Skills and Abilities: _ Ability to effectively present information and respond to questions from clients, office personnel, other employees and the general public Ability to read, analyze and interpret manuals and office documents_Mental/Interpersonal Skills and Abilities: _ Ability to define problems, gather, transcribe and post data, analyze, synthesize and classify information, and draw valid conclusions Ability to work independently and with others - including other employees, clients and members of the public in face-to-face and telephonic contexts Ability to multi-task, establish priorities and manage time, meet deadlines and perform duties under time constraints Ability to be flexible and adapt to changing work demands Maintain high level of concentration and attention to detail for extended periods of time Ability to respond effectively to sensitive inquiries or complaints Maintain a high level of ethical and professional standards in accordance with agency and community policy_Physical Skills and Abilities: _ Ability to talk or hear in order to give and receive information and instructions Ability to stand, walk, sit, use hands to finger, handle or feel objects, tools or controls Ability to reach with hands and arms Ability to use computer keyboard up to 50% of the day Visual acuity, ability to adjust focus and peripheral vision for work on a laptop or other computer equipment Ability to lift and/or move up to 20 pounds_Reasonable accommodation for individuals with disability will be evaluated and exceptions considered.__Equipment and Computer Software Skills and Abilities: _ Computer literate (basic working knowledge of Microsoft WORD, Excel, and Outlook) Able to use telephone, typewriter, office copier, calculator, fax machine, computer printer and scanner, or other equipment as required EDUCATION, EXPERIENCE, CERTIFICATES & TRAINING Bachelor's degree or Associates degree with 1-2 years of experience in a relevant social service setting; Experience working with transitional age youth, individual with mental disabilities, low income families. Demonstrated ability to work with a culturally diverse clientele & staff Valid California driver's license and personal vehicle insurance carrier relating to clients, staff agency representatives, community members and others. _This position will require you to drive your personal vehicle for work._ ADDITIONAL REQUIREMENTS Must be available to work flexible hours in the evenings during the week.Level of experience and educational level will be taken into consideration. This is a full-time position. Health, dental and life insurance provided. Must have your own car, valid driver license and car insurance.
