Job Title: Post-Acute Care Business Development & Navigator Manager The post-acute care navigator promotes hospital services and specifically Pipeline sub-acute units to the SNF, Board & Care, Assisted Living communities and the health care provider community at large. The navigator serves as liaison between Hospital Administration and the Medical Staff focusing on nurturing and building relationships with internal and external referral sources. Serves as a role model and provides expertise on Post-Acute Care program and provides smooth transition for patients and families by coordinating care with case managers, nursing staff, social workers, physicians and vendors. This position reports to Business Development DUTIES AND RESPONSIBILITIES Promote post-acute units to acute, post-acute facilities and other health care providers.Maintain a consistent channel of communication between the various hospital departments.Research prospective referral sources in target market and pursue leads.Understand the post-acute environment which market strategies can be used to build relationship and referral programs.Maintain relationships with current clients and identify new prospects within the area you have been assigned.Possess a strong understanding of hospital services, our competition in the industry and positioning.Follow the latest industry developments and stay up-to-date on competitors.Communication/Knowledge: Effective written and verbal communication skills; able to communicate with various individuals both inside and outside the organization; able to understand and create reports/presentations/proposals.Ability to work under stressful situations.Proficient with computer applications and systems.Must have understanding of marketing to post-acute care community. Possess knowledge of promotional programs and community relations relating to healthcare industry The following job accountabilities are not unique to this particular job but are common to all jobs at Pipeline Health Hospitals: Complies with Pipeline Health policies and procedures on customer satisfaction and service excellence. Works effectively and collaboratively with others toward common goals.Communicates accurately, honestly, supportively and in a timely manner with department and interdepartmental team members. Demonstrates effective business writing and oral communication skills, handwriting is clear and legible.Participates in operational aspects of the department and maintains/participates in performance improvement activities within the department.Participates in all departmental specific training,Demonstrates knowledge of and follows safety practices.Actively participates in the Patient Safety Program, including event reporting. Identifies sentinel events/near misses and responds per defined organization processes. Participates in education activities and process implementation.Demonstrates advocacy for the patient/customer and appropriately acknowledges patients, customers and visitors.EXPERIENCE/QUALIFICATIONS: Clinical background, preferred RNBachelor's degree in related field preferred, or equivalent experienceMinimum of 5 to 10 years experience in business development experience.
